Training Support Service Log

Participant Responsibilities:

• Name: Fill in your full name in the “Participant Name” field.
• Date(s): Record the date(s) of the activity in the “Date” column.
• Total Hours: Calculate the total hours spent on each activity (in 0.25-hour increments) and enter it in the “Total Hours” column.
• Signature: Provide your signature in the “Participant Signature” field.
• Signature Date: Write the date you signed the form in the “DATE” field below your signature.

Submitting the Log:

• After completing your section, submit the log to the Employment Service Specialist for further processing.

Employment Service Specialist Responsibilities:

• Verify the details entered by the participant.
• Complete the remaining fields in the form, including details of the activities and topics covered, case ID number, service provider name, authorization issue date, and any additional required details.
• Calculate the total hours billed across all activities.

Final Submission:

• The Employment Service Specialist must submit the completed Training Support Services Log, along with a Provider Summary Invoice, to the AWARE Portal.
